Careful follow-up and monitoring after hospital or rehabilitation stays to ensure a safe transition home and prevent readmission.

Optimizing medications for safety, reducing unnecessary prescriptions, and performing in-home diagnostics and minor procedures.
Care Coordination
Working closely with specialists, home health agencies, physical therapists, and hospice to ensure all aspects of your care are aligned.
We proudly accept Medicare along with many other insurance providers. However, we currently participate with only a limited number of Medicare Advantage plans.
A monthly travel fee ($75) applies for patients living in private homes and not in a senior living facility.
